HMG: what the research covers

A preparation rather than a molecule. It carries FSH and LH-like activity in the same vial, which is both what makes it useful and what makes it harder to reason about.

What HMG is

Human menopausal gonadotropin is purified from human urinary gonadotropins and standardised by biological activity, conventionally at seventy-five international units of FSH activity per vial with a comparable LH-like activity alongside it. A measurable part of that LH-like activity is contributed by HCG present in the source material.

So this is not one compound. It is a defined-activity preparation, and the specification is in international units because mass would not tell you what the vial does. Where HCG is a single glycoprotein acting at a single receptor, HMG engages two.

FSH acts at FSHR on granulosa and Sertoli cells; LH and HCG act at LHCGR on theca and Leydig cells. Both arms are present here, which means both cell populations in a co-culture respond at once.

Why a mixed preparation is sometimes the right tool

Follicular development requires both receptor arms: the two-cell two-gonadotropin model has theca cells producing androgen under LH and granulosa cells aromatising it under FSH. A preparation containing both reproduces that arrangement in a way that a single recombinant hormone cannot, which is why menotropin remains in use in follicular culture work despite recombinant alternatives existing.

The corresponding weakness is attribution. An effect observed with HMG cannot be assigned to FSH or to the LH-like component without a comparison, and the standard design is therefore HMG against recombinant FSH alone, with the difference attributed to the LH arm. Anyone working with HMG without that comparator is measuring a combined response and should describe it as one.

A third strand of the literature is analytical: batch-to-batch variation in isoform composition and specific activity is an acknowledged property of urinary-derived preparations, and characterising it is a research subject in its own right.

The certificate for a biologically standardised preparation answers different questions from one for a synthetic peptide: activity and composition rather than purity of a single sequence. That is a genuine limitation and it is better stated plainly. Handling follows glycoprotein practice — cold storage of the sealed vial, small working volumes, no vigorous agitation, no repeated freeze-thaw cycling, since activity degrades before protein content does. Diluents are under reconstitution supplies.
