Ipamorelin: what the research covers

A pentapeptide ghrelin receptor agonist, studied because it is the most selective of the secretagogues rather than the most potent. What that selectivity means in practice.

What Ipamorelin is

Ipamorelin is a synthetic pentapeptide — Aib-His-D-2-Nal-D-Phe-Lys-NH2 — that acts as an agonist at the growth hormone secretagogue receptor, GHSR-1a. That is the receptor for ghrelin, and it is a different target from the GHRH receptor that CJC-1295 and Sermorelin act on.

Its significance in the literature rests on selectivity. The earlier secretagogues of this class — GHRP-2 and Hexarelin — produce measurable increases in cortisol, prolactin and ACTH alongside growth hormone release. Ipamorelin was developed specifically to reduce that cross-activity, and at research concentrations it produces substantially less of it. It is less potent as a releaser than either, and that trade-off is the entire point of the compound.

Why selectivity matters to a study design

A compound that raises cortisol alongside growth hormone confounds any endpoint that cortisol also affects — which is a long list, covering metabolic, immune and behavioural readouts. In a study attempting to attribute an observation to growth hormone signalling specifically, a cleaner releaser is worth more than a stronger one.

The corollary is that where maximum release is the objective and off-target activity is tolerable or is itself the subject, GHRP-2 or Hexarelin are the more appropriate choices. The Ipamorelin versus GHRP-2 comparison sets out where each is the better fit.

Because GHSR-1a and the GHRH receptor are distinct, agonists at the two are frequently combined to produce a larger response than either gives alone; that is what the CJC-1295 / Ipamorelin preparation is for. Literature is indexed on PubMed.

The sequence contains two D-amino acids and a C-terminal amide, none of which are default outcomes of peptide synthesis, so identity confirmation is doing real work here rather than serving as a formality.
