Hexarelin: what the research covers

A hexapeptide secretagogue that binds two unrelated receptors. The second one, CD36, is why hexarelin has a cardiac literature that none of its siblings share.

What Hexarelin is

Hexarelin is His-D-2-methyl-Trp-Ala-Trp-D-Phe-Lys-NH2. The methyl group on the second tryptophan is the stability feature; the D-configuration residues and the terminal amide do the rest of the work of surviving in solution.

At GHSR-1a it is a potent agonist, comparable to GHRP-2 in the size of the growth hormone response it produces. It also desensitises quickly under continuous exposure, which is a property of the receptor rather than of this particular ligand, but hexarelin is where it was characterised most thoroughly.

Two receptors, two literatures

What sets hexarelin apart is that it binds CD36, a scavenger receptor expressed on cardiac myocytes, macrophages and endothelium, which has nothing to do with growth hormone. That second interaction produced a body of cardiovascular work — infarct models, myocardial protection, angiogenesis — in which the effects persist in animals lacking a functional growth hormone axis.

For an experimentalist this is both an opportunity and a trap. The opportunity is a single ligand that can be used to interrogate two unrelated pathways. The trap is attributing a cardiac observation to growth hormone release when CD36 is the more likely route, which is exactly why hexarelin studies so often include a selective secretagogue such as ipamorelin as a discriminating control.
